1.14.1 INCIDENT REPORTING PROCEDURES

An air traffic incident is an occurrence other than an accident associated with the operation of an aircraft which affect or could affect the safety of operation.

Pilot shall file all incident reports on CAA form "Pilot report on Communications, Air Traffic Control and Meteorological services" in order to speed up the process of investigation of the various categories of incidents.

1.14.2 REPORTING OF AIR TRAFFIC INCIDENTS BY PILOTS

A pilot should proceed as follows regarding an incident involved:

during flight, use the appropriate air/ground frequency for reporting an incident of major significance, particularly if it involves other aircraft, so as to permit the facts to be ascertained immediately.

as promptly as possible after landing, submit a completed "Pilot report on Communications, Air Traffic Control and Meteorological services":

for confirming a report of an incident made initially as in a. above, or for making the initial report on such an incident if it had not been possible to report it by radio.

for reporting an incident which did not require immediate notification at the time of occurrence.

The CAA form "Pilot report on Communications, Air Traffic Control and Meteorological services" should be submitted to the relevant Flight Information Station (FIS) or to Flight Operation Section for airports without FIS.

1.14.3 FAULT REPORT ON COMMUNICATION AND NAVIGATION FACILITIES

In order to enable the CAA to closely monitor the performance of its communication facilities, visual and non-visual aids, and to undertake remedial action immediately on any faults occurring, pilots operating within the Taipei FIR are kindly requested to bring to the notice of the CAA immediately any faults that are detected by them. Pilots are requested to use of CAA Form "Pilot report on Communications, Air Traffic Control and Meteorological services" in making their reports.

Pilots may hand in the form to the relevant FIS or to Flight Operation Section for airports without FIS.

1.14.4 AVAILABILITY OF CAA FORM

The above-mentioned CAA Forms are available at the FIS of Taipei/Taiwan Taoyuan International Airport, Kaohsiung International Airport and Taipei/Songshan Airport, or at Flight Operation Section for airports without FIS.

The duty officer of FIS and Flight Operation Section, on receipt of the report will follow the procedure and forward it as soon as possible to the agency concerned.

All Incident Reports filed will be thoroughly investigated and the complainant will be notified of the results of the investigation as soon as possible.
